IMPORTANT!! Please review this information completely!!
This is a drive through clinic. You will stay in the car for the duration of this clinic. Our nurses will administer shots while the patient is in the vehicle. We will be providing Meningitis (MCV4) and Men B vaccinations at this clinic. No other immunizations will be provided.
Please bring the following items with you:
1. Your child’s immunization record. This is required.
2. Intake forms. You can download the patient forms here.
3. Everyone in the car is expected to wear a mask.
