Must bring your Child's Immunization Record
Child must be accompanied by parent or guardian.
If your child has private insurance that covers vaccines he/she is not eligible for free vaccines.
Children with Medicaid are eligible for this service.
Note: The Care Van Program will offer influenza vaccine while supplies last. Not all clinic locations listed above will offer influenza vaccine but will continue providing childhood immunizations.
